Martial Arts Participation Waiver and Liability Release
I recognize and understand that martial arts training is a physical contact activity. I acknowledge that my participation may result in serious injury, including permanent disability, death, and significant social or economic loss. I further understand that such risks may arise not only from my own actions, but also from the actions, inactions, or negligence of others. I recognize that there may be additional risks that are unknown or not reasonably foreseeable at this time.
I agree to inspect the facilities, equipment, and training pairings prior to participation. I will immediately notify an instructor if I believe any condition or activity is unsafe or beyond my capability, and I will refuse to participate in such circumstances. I voluntarily assume all the above risks and accept full personal responsibility for any injury, permanent disability, death, or other damages that may result from my participation.
I enter martial arts training and/or competition entirely of my own free will and understand the importance of following all training and competition rules. I certify that I am in good physical condition and have no medical condition, injury, or disease that could impair my performance or physical and mental well-being during training or competition. If I have any injuries or illnesses that could affect my ability to participate, I will notify the club managers and coaches, and consult with my or my child’s doctor regarding the appropriateness of martial arts training.
I grant permission for doctors, nurses, athletic trainers, or other emergency medical personnel to provide medical assistance or treatment in case of injury. I understand that all costs related to treatment or hospitalization arising from injury during training or competition shall be my responsibility and not that of Superior Taekwondo, its affiliated organizations, governing bodies, officers, instructors, personnel, other members, participants, supervisors, coaches, sponsoring organizations, or their agents.
I hereby release, waive, discharge, and covenant not to sue Superior Taekwondo, its affiliated organizations and governing bodies, their officers, instructors, personnel, other members, participants, supervisors, coaches, sponsoring organizations or their agents, and, if applicable, owners and lessors of the premises, from any and all liability, claims, demands, losses, or damages arising from injury, death, or property damage, whether caused in whole or in part by their negligence or otherwise.
